How to Register as a Patient

If you would like to register as a patient at this Practice, please collect a 'New Patient Pack' from the Surgery.  The Pack contains:


a)   GPR (Registration Form)

b)   New Patient Questionnaire

c)   Practice Leaflet



Alternatively, you can download a GPR Form and New Patient Questionnaire from the 'Our Documents' section in the right margin of this page,


Please complete one GPR and one New Patient Questionnaire for each person registering with the Practice.


If you are newly registered with our Practice, you will be offered a new patient appointment with one of our Practice Nurses. This allows us to get to know you and your past medical history and treatment needs while we await your medical records arrivinge from your previous doctor.

Please bring the following to your appointment:


a) Completed GPR (Registration Form)

b) Completed New Patient Questionnaire

c) Proof of identity


If you are taking any long term medication it is also helpful if you could bring a list of your current medicines with you.  The reorder portion of your current prescription is ideal and will give us the information we require. 


Under the current contract for general practice, you will not be registered with a specific doctor. Instead, you will be registered with Cathcart Street Practice.


It is very important that you let us know if you are unable to attend your Registration appointment for any reason.  Patients who fail to attend their Registration appointment will not be accepted on to our Practice List.
